I/We, the legal guardian(s)/parent(s) of the minors registered for the RE program, give our permission to NUUC for our children to participate in the RE program during the 2023-24 church year.

I/We release NUUC for any liability of claim or cause of action that I may now have, or may have in the future as a result of any injuries received by the said child(ren) while s/he/they participating in any NUUC activities.

In the event of an accident, I understand that an attempt will be made to contact me at the phone numbers provided on the Registration Form. If the emergency contact, or I/We, cannot be reached in time,  I hereby authorize the administration of emergency medical aid to my child(ren). I agree to accept financial responsibility for any and all emergency medical care so provided.

I/We agree to accept responsibility for the supervision of our young people while they are using electronic devices.

I/We understand that NUUC is a public space and cannot be held responsible for photos or videos taken and used by non-staff members outside of official NUUC publication spaces.

I/We understand that NUUC staff and RE volunteers are mandated reporters and cooperate with all community safety reporting practices.

I/We acknowledge that NUUC requires adherence to our Safe Congregation policy for official NUUC activities and understand that it contains recommended practices for unofficial activities as well.

I/We commit to honoring the NUUC Covenant of Right Relations and all NUUC policies and procedures.

Northlake's Religious Education program is supported by the whole congregation and therefore there are no registration fees required for participation.
